There are pockets of competitive pricing for meds, tests and procedures but patients won't be told about them because most physicians are too busy, don't know how to shop for these themselves or figure that's somebody's else's job. It's hard to compare prices unless you know where to look because, until recently, very few people paid for these themselves but bought via insurers and government agencies so the pharmacies don't seem motivated yet to compete on price on most meds.

Insurance companies insulate (for a price) the consumer from the provider.
